Partners Home Health Care Inc. is a home health agency providing QUALITY, SAFE and COST-EFFECTIVE home care services. We provide comprehensive, quality, patient-centered care that is focused on each patient’s needs. We endeavor to be the provider of choice for home care services: intermittent skilled nursing, physical & occupational therapy, speech language pathology, dietary consultation, home health aide and medical social services.

Nursing care depends on the person's needs. In consultation with the doctor, a registered nurse will set up a plan of care. Nursing care may include wound dressing, ostomy care, intravenous therapy, administering medication, monitoring the general health of the patient, pain control, and other health support.
Some patients may need help relearning how to perform activities of daily living or improve their speech after an illness or injury. A physical therapist can put together a plan of care to help a patient regain or strengthen use of muscles and joints. An occupational therapist can help a patient with physical, developmental, social, or emotional disabilities relearn how to perform such daily functions as eating, bathing, dressing, and more. A speech therapist can help a patient with impaired speech regain the ability to communicate clearly.
Medical social workers provide various services to the patient, including counseling and locating community resources to help the patient in his or her recovery. Some social workers are also the patient's case manager--if the patient's medical condition is very complex and requires coordination of many services.
Home health aides can help the patient with his or her basic personal needs such as getting out of bed, walking, bathing, and dressing. Some aides have received specialized training to assist with more specialized care under the supervision of a nurse.
Raymond has worked for home health since 1996 – from a quality assurance reviewer to an assistant administrator until he became a health administrator in 2006. He is a medical doctor by profession but immigrating to the US, he has devoted his career in caring for the elderly in a home setting. He has obtained a certification in Medicare coding. Raymond has likewise been a consultant for numerous home health agencies in California. His clinical skills and experience in home health and hospice makes his qualification unique. He is most effective in coordinating care with all disciplines involved and focusing on each patient’s distinctive needs.
Rosario has a law degree and completed her Masters in International Law with concentration in Human Rights and Humanitarian Law. She brings the heart into the organization. Her passion to advocate for basic rights and the elderly started when she worked for the United Nations and in 1996 when she started working for a home health agency. She has been a health administrator since 2000. To date she has been a consultant in setting up home health agencies and continues to assist agencies achieve compliance for survey. Her organizational skills coupled with her love to teach and mentor community liaisons is adequate to promote the welfare of the aged.
